Welcome to the Glow Design Group crew page. If you have already worked for us please follow the links below to download the necessary forms. Please fill out both the I9 and W4 forms and mail them back to us along with a CLEAR copy of either your drivers license and Social Security Card, or your passport. Please also download and fill out the new employee form and mail that back as well.
If you haven’t worked for but would like to please send us a resume and cover letter to the email address below and we will add you to our crew list. PLEASE DO NOT CALL US ON THE PHONE
Booking
All calls are booked via e-mail and are available on a first come first served basis. When we get a call an e-mail is sent to our entire list. Please reply if you are available and we will let you know job details if there are still available slots. If you would like to be removed from our crew list send and email with “remove” in the subject line. If you are leaving town and want to be temporarily removed send and email with “out of town” in the subject line. When you want back on the list send one with “back in town”
Payroll
All payroll is run through our payroll department. Time-sheets must be filled out and submitted by Monday. You can email, mail or fax your time-sheet. Payroll is entered bi-weekly and checks are sent out every other Thursday. Checks MUST be delivered via USPS they cannot be picked up. Pay periods run through Sunday and checks are cut and sent out for the previous two week pay period every other Thursday.
If you believe your check to be incorrect please send us an email and let us know what hours you worked and we will double check them with our time-sheets.
The Timesheet can be downloaded below. This form can either be printed and faxed or mailed back, or can be filled out electronically using the free adobe reader available at www.adobe.com. The completed time-sheet can then be e-mailed back to crew@glowdesigngroup.com
